WHO IS ELIGIBILE :

• family meets federal income guidelines OR
• child has a diagnosed disability for Head Start OR
• child is a military dependent (4 year old Pre-k only) OR
• child is homeless OR
• child is in or has been in CPS conservatorship OR
• child is a limited English speaker for Pre-K (4 year old Pre-K only)

REQUIRED DOCUMENTS

• Child’s Birth Certificate
• Immunization Record
• Social Security Card
• Proof of Residency: the following will be accepted: utility bill (elec., gas, and water), rent receipt, lease or mortgage.
• Proof of income for ALL ADULTS living in the home for the last 12 months (check stubs AND filed income tax for (2018) OR W-2, ALSO BRING IF APPLICABLE Social security printout, TANF, letter from employer, child support, financial aid, etc.).
• Military ONLY 4 year olds are eligible for Pre-K: Child’s DOD ID card, sponsors orders ( no income required )
• Picture ID

Early Head Start – Birth to 3
Head Start – 3 years old on/before Sept. 1, 2019 (limited space)
Head Start – 4 years old on/before Sept. 1, 2019
Pre-K – 4 years old on/before Sept. 1, 2019
