To register your child, birth to 3 years old, for the 22-23 school year:

 Please bring the following with you:

• Your child’s birth certificate
• Your child’s social security card
• Your child’s shot record
• Your child’s new-born hearing results
• Your proof of address (Must be a Lease agreement or a utility bill with your name on it)
• Proof of income of any parents in the household (Income tax or W2 is best.  If you bring check stubs, we will need 12 months worth).  Also if you receive SSI, SNAP or TANF we will need a copy.
• Your picture ID
