If your card has been found, lost, or stolen immediately complete the Reporting a Lost ID form available in eduphoria formspace.

To submit a form in eduphoria Formspace follow the below steps.

Direct a browser to eduphoria by typing https://eduphoria.abileneisd.org in the address bar.
Login using your district username and password.
Select Formspace.
In the bottom left hand corner, select Submit New Form.
Select ID Badges
Select the appropriate form.
When all information has been completed, select submit form at the bottom of the form.

 If you are unable to log into eduphoria and complete the form please email LostID@abileneisd.org immediately.
